To be eligible for athletics at PSHS, all forms must be completed at Student Central (formerly known as PlanetHS) PRIOR to the first day of practice.  In addition to completing all of the forms online, a current physical must be uploaded to the site, as well.  Physicals are good for 365 days from the date of the examination.  If your current physical is still valid, you will be able tto select that physical from last year and roll it over to this year.  If it has expired, or will expire during your season, you will need to upload a new physical examination form.
